- 18 years old or older
- Meet ASAM criteria for level 3.5 residential treatment
- Meet substance dependence (ICD-10) for at least one substance
- No convictions that resulted in being placed on the sex offender registry
- Have or be eligible for KY Medicaid, or have active third-party insurance within the Genesis network
- Stable medical condition lacking the need for immediate or urgent medical care
